360 Degree Feedback Survey 2007 (Faculty of Education, Monash)

Welcome to the Monash Faculty of Education 360° Feedback website. This facility enables its participants to confidentially fill in a 360° scale which will assist them to better understand their strengths and areas they need to work on.

Your surveys are as follows:

Performance Feedback (Self) survey - to be completed by the participants of the course.
Performance Feedback (Other) survey - to be completed by the person approached by the participant.

You will need two pieces of information in order to complete the survey:

the survey password to enter the survey, and
the participant ID, i.e. a unique number assigned to each participant.

These pieces of information will be provided to each participant in the mail.
